Purpose
To provide a continued educational opportunity to individuals who demonstrate a strong commitment to philanthropic excellence and community involvement.

Eligibility Criteria

Must be a Member of SHAPE for at least one complete year prior to applying for the SHAPE scholarship
Applicants shall have demonstrated commitment to their community. The candidate should articulate what he/she has done and learned in the community that would fulfill this criterion
Applicant has demonstrated strong leadership skills. Specify how this has been achieved
Applicant has received recognition for noteworthy accomplishments
Applicant has achieved a 2.7 Grade Point average or better
Applicant must be matriculating into college, university or technical school in the year this grant is awarded

Award Information
One student will be chosen to receive a $5,000 scholarship to the college, university or technical school of his/her choice. The award can be renewed on a yearly basis for up to three additional years after a review of the student’s progress based on the renewal funding criteria.

Application Procedure
Complete packets must be received by March 9th and the recipient of the award will be announced on April 23rd.
The following should be sent in one complete packet to the SHAPE Administrator’s office:

Completed application form
Personal statement from the applicant to explain his/her suitability for the award. The personal statement should address how the applicant fulfills the purpose and criteria for the award. The letter can be no longer than two pages, double spaced with a 12pt font
Letters of recommendation/support from two references. References should be provided by people familiar with the applicant who can speak to the applicant’s suitability for the award as defined by the Purpose and Criteria.
A letter of acceptance to an accredited college, university, or technical school should be included in the packet. If candidates have not yet received a letter of acceptance at the time of the application, they should indicate the intended school of attendance and provide a letter as soon as available from the college’s Office of Admission.
Proof of SHAPE membership (Letter to be provided by SHAPE advisor and indicate number of years and level of participation)
Official transcript to be provided by the applicant’s school

Selection and Announcement
All complete application packets will be reviewed and rated against the eligibility criteria by the SHAPE Selection Committee. The selection committee will look for documentation that supports the application. Applicants may be requested to appear for a personal interview. The recipient will be notified by April 23rd. The scholarship funds will be paid directly to the college, university or technical school that the recipient attends.

Renewal Funding Criteria
Renewal of funding will be contingent on:
1. A letter from the registrar, or other appropriate institutional official, indicating that the recipient is a student in good standing and the student’s official grade point average. Students must maintain a minimum of a 2.7 GPA for continuation funding
2. Documentation of participation in a community social service or volunteer program. A letter from the director of the program or the recipient’s supervisor should be provided
3. The above should be provided after the completion of the academic year and prior to the beginning of the following year to assure that the student’s entire body of work for the academic year is complete and can be reviewed by the committee
